pearls

  • The use of this score requires that the user understand ultrasound terminology and findings.

usecase

  • In patients who are undergoing adnexal surgery. 
  • Not for use in patients who are being treated with observation or other conservative measures. 

reasons

The original Simple Rules study provided a list of 5 benign characteristics and 5 malignant characteristics. An ultrasound report was classified as malignant or benign if findings met all 5 characteristics in one of the groups. Ultrasound reports which showed a mix of these characteristics were classified as “indeterminate” which limited the score’s clinical use. Study authors created a logistic regression model in 2016 and added another input; the location where the ultrasound imaging was performed. This enhanced model with risk calculation is what MDCalc has chosen to include.

  • M features are more closely associated with malignancy while B features are more associated with a benign tumor. 
  • The presence of M features increases the risk of a tumor being malignant while the presence of B features will decrease the risk. 
  • Use as part of presurgical counseling and planning with your patient. Remember to only use this score in patients who are undergoing surgery.
