Step 1 - Severity Classification

The disease severity classification utilized by this tool (Mild, Moderate, Severe, Critical) is based upon that adopted by the National Institute of Health COVID-19 Treatment Guidelines Panel which in turn is based upon an early consensus of the literature describing the disease entity initially presenting in Wuhan, China, and subsequently identified as SARS-CoV-2.  (https://www.covid19treatmentguidelines.nih.gov/).

Step 2 - Diagnostic Testing and Interpretation

This section follows NIH guidelines for testing recommendations that are maintained here: (https://www.covid19treatmentguidelines.nih.gov/overview/). Recommendations within this section will be updated as NIH makes updates to these recommendations. Evidence for exertional O2 measurement is included from (Greenhalgh 2020, Goodacre 2020, Paglia 2020).

It is recommended to utilize ACEP’s COVID-19 Field Guide section on abnormal lab values to review lab results.

Step 3 - Disposition

This section lays out appropriate disposition of the patient based on their severity classification. Most of this section was devised from consensus among the working group. Supplemental evidence was included from Banerjee 2021, links to patient educational materials from the CDC (https://www.cdc.gov/coronavirus/2019-ncov/if-you-are-sick/index.html), and multiple helpful links for patients from JAMA (example: https://jamanetwork.com/journals/jamapediatrics/fullarticle/2763176).

Step 4 - Non-Pharmacologic Treatment

This section cites 8 references which include non-pharmacologic treatment of respiratory infections in general (e.g. Patchett 2021) as well as COVID-19 specifically (e.g. Banerjee 2021). It also includes a review of treatments for outpatient and inpatient COVID-19 management (https://www.covid19treatmentguidelines.nih.gov/management/). The recommendations are listed, based upon provider assessment of disease severity. Some of these recommendations are consensus-based.

Step 5 - Pharmacologic Treatment

This section is abstracted directly from the NIH COVID-19 Treatment Guidelines (https://www.covid19treatmentguidelines.nih.gov/therapies/), which are mainly evidence-based. The entries are listed by severity of disease. When provided in the NIH guidelines, the strength of the recommendation and level of evidence for the recommendation are specified. For example, for moderate, severe and critical COVID-19, the following is provided:

  • Remdesivir: For hospitalized patients who require minimal supplemental oxygen (BIIa). 

Optional Step - Risk Prognostication

This step utilizes the PRIEST Score as a validated tool to predict a patient’s risk for end organ failure and/or mortality. It was a mixed prospective and retrospective observational cohort study in 70 emergency departments across the United Kingdom (UK). 22,445 patients were included who were suspected to have COVID-19 between 26 March 2020 and 28 May 2020. This tool was included because it did not require diagnostic testing (labs, imaging) as part of the evaluation of the patient. (https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7822515/).

Optional Step - Risk Assessment

Additional risk factors that could increase the patient’s risk for severe disease, organ failure, and/or mortality can also be considered when evaluating a patient with COVID-19. The CDC maintains information that is consistently updated with data for risk associated with race/ethnicity, socioeconomic status, and healthcare resources (https://www.cdc.gov/coronavirus/2019-ncov/covid-data/investigations-discovery/hospitalization-death-by-race-ethnicity.html). Additional references (Bellou 2020, Ebinger 2020, Lokken 2021, Strausz 2021, Williamson 2020, Tartof 2020) are included that summarize evidence for other risk factors.

This algorithm provides a pragmatic framework for standardizing severity classification, risk assessment, diagnostic workup, disposition, and treatment of adult patients  with suspected or confirmed SARS-CoV-2 (COVID-19).

Patients presenting with symptoms conditions that may be manifestations of severe or critical COVID-19 may also be manifestations of other diseases (e.g. MI, PE, stroke), requiring additional specific diagnostic and therapeutic interventions.

next_actions

  • There is no need to apply this tool to patients who are not being evaluated for COVID-19.

  • If the patient is considered mild disease, this tool may help avoid further testing.

  • Imaging, lab tests, treatment and disposition should be considered based on severity and risk for disease progression.
