This score is not diagnostic for polymyalgia rheumatica but can be used for research purposes and assistance in classification of symptoms.

usecase

This score can be used to classify patients who present with symptoms concerning for polymyalgia rheumatica (PMR).
It should only be used with patients who are ≥50 years of age, present with bilateral shoulder pain, and who have an abnormal ESR and/or CRP.
You can use this score with or without ultrasound findings, but the cutoff value for classification will change.

next_advice

Patients scoring ≥4 without ultrasound are classified as having PMR.
Patients scoring ≥5 with ultrasound are classified as having PMR.
Initial therapy usually includes oral steroids but clinical judgment should be used when determining treatment pathway.
